ABSTRACT

We herein present the case of a patient with anal condylomatosis concomitant with histoplasmosis, whose diagnosis was only possible through the collection of material and the subsequent evidence of a primary pulmonary focus. Histoplasmosis is a fungal disease whose contamination occurs through the respiratory route, and it can spread to the digestive tract, but the anus is rarely affected. It is important to have a high degree of suspicion to make the diagnosis, especially in immunosuppressed patients.

ABSTRACT

Introducción: El condiloma acuminado gigante es una rara enfermedad que se manifiesta en el 0,1 % de las personas con vida sexual activa, su tratamiento es variable; la cirugía es el más radical y definitivo, aunque la recurrencia no es despreciable. Ante grandes defectos de la región perianal luego de la resección es útil la rotación de colgajos para recubrir el defecto. Objetivos: Exponer el tratamiento quirúrgico reconstructivo realizado a un paciente con tumor de Büschke-Löwenstein perianal. Caso clínico: Paciente masculino de 47 años, afroamericano, con antecedentes de infección por el VIH, diagnosticado 8 meses antes de la consulta por el cuadro actual; en tratamiento con antirretrovirales. Acude a consulta con una lesión anal en forma de coliflor, que ocupa un diámetro de aproximadamente 10 cm; se le realizó un procedimiento quirúrgico para tratamiento definitivo, mediante escisión y anoplastia con colgajo en V-Y y se extirpó a lesión vegetante sobre el borde interno del glúteo derecho, cuya área formó parte del colgajo V-Y. Conclusiones: Las técnicas reconstructivas en la región perianal se deben individualizar en cada caso, en dependencia del tipo de defecto, su topografía y las condiciones locales; aunque pudiese existir la preferencia de una sobre otras, la literatura científica no aporta suficiente evidencia al respecto. La técnica reconstructiva realizada al paciente demostró que, aun con altas tasas de recurrencia y complicaciones, resultó efectiva para el paciente pese a tener comorbilidades que pudieron influir en una evolución tórpida.


Introduction: The giant condyloma acuminata is a rare disease that manifests itself in 0.1% of people with an active sexual life, its treatment is variable, surgery being the most radical and definitive, although recurrence is not negligible. In case of large defects in the perianal region after resection, flap rotation is useful to cover the defect. Objectives: To expose the reconstructive surgical treatment performed on a patient with perianal Büschke-Löwenstein tumor. Clinical case: A 47-year-old African American male patient with a history of HIV infection diagnosed 8 months prior to the current consultation, in treatment with antiretrovirals. Who came to the clinic with an anal lesion in the shape of a cauliflower, which occupies a diameter of approximately 10 cm, a surgical procedure was performed as a definitive treatment through excision and anoplasty with a V-Y flap, and the vegetative lesion on the edge was excised, internal of the right gluteus whose area was part of the V-Y flap. Conclusions: Reconstructive techniques in the perianal region must be individualized in each case, depending on the type of defect, its topography, and local conditions, and although there may be a preference for one over others, the scientific literature does not provide sufficient evidence in this regard. The reconstructive technique performed on the patient showed that even with high rates of recurrence and complications, it was effective for the patient despite having comorbidities that could influence a torpid evolution.

@#Abstract: Objective The purpose of this study is to find out the epidemiological and clinical characteristics of male patients with condyloma acuminatum (CA) in Liaocheng area, as well as to improve the understanding of human papillomavirus (HPV) infection and put forward targeted prevention and treatment measures. Methods The clinical data of 159 male CA patients who admitted to Liaocheng People's Hospital from January 2021 to January 2022 were collected, and the epidemiological characteristics, clinical manifestations, infection sites, HPV gene subtypes and other information were analyzed retrospectively. Results Most of the 159 CA patients (range from 15 to 77 years old) were 31-40 years old (31.45%, 50/159), more than half of them had smoking history, and more than 60% had low income (<5 000 yuan/month), multiple sexual partners (≥2) and no condom use habit, 70.44% of the patients had prepuce long combined with prepuce balanitis. 91 cases (57.23%) were single infection, 102 cases (64.15%) were simple low-risk HPV infection. The analysis of risk factors between mixed infection and simple low-risk infection found statistically significant differences in age≤ 40 years old, unmarried, or less affected by education duration of 15 years or less, engaged in the business or service industry, the number of sexual partners or 2, knew not to clean and not to use condoms, while differences in smoking history, alcohol history, monthly income level, and age at first sexual intercourse were not statistically significant. Low-risk HPV6 and/or HPV11 were detected in 139 cases (87.42%). Fifty-seven patients (35.85%) were infected with at least one high-risk HPV. 72.33% of the patients had multiple warts, and the most common sites were around the coronal sulcus and frenulum of the penis Conclusions The incidence of multiple infections and high-risk subtypes is high in male CA patients in Liaocheng area, and most of the patients have low income, low education level and multiple sexual partners. Strengthening the treatment and education follow-up of this population may contribute to the treatment and prognosis of male CA in this area.

Objetivos: A infecção pelo papiloma vírus humano dos tipos 6 e 11 pode evoluir para Condiloma Acuminado. Sua transmissão é principalmente via sexual, de forma que a presença dessas verrugas em crianças deve ser consi-derada sinal de alarme para investigação de abuso. Eventualmente, contudo, ocorre por autoinoculação ou fômites. O tratamento é desafiador, dado que apesar do extenso arsenal, a recorrência se faz comum. A presente revisão siste-mática advém a comparar taxas de sucesso das estratégias terapêuticas, objetivando fornecer substrato para escolhaacertada no tratamento da doença em crianças e adolescentes. Métodos: Foi realizada Revisão Sistemática baseada no GuidelinePreferred Report Items for Systematic Reviews and Meta-Analyses (PRISMA), registrada na Plataforma PROSPERO sob identificação CRD42021262090. Resultados: 211 estudos foram encontrados nasdatabases con-sultadas, de forma que 30 atenderam as propostas de critérios estabelecidos na pesquisa sistematizada. Esses pas-saram extração de dados para síntese qualitativa, demonstrando eficácia de métodos não-invasivos a invasivos.Conclusões: Mediante bons resultados em resolução da doença e reduzidos efeitos adversos, a terapia mais popular entre os autores foi o Imiquimode. A pesquisa chamou atenção à necessidade de estudos mais robustos e metodo-logias mais complexas acerca das abordagens terapêuticas do Condiloma Acuminado em crianças e adolescentes. (AU)


Objectives: Infection with human papillomavirus types 6 and 11 can progress to Condyloma acuminata. Its transmission is mainly sexual, so these warts in children should be considered an alarm signal for abuse investigation. Eventually, however, it occurs by autoinoculation or fomites. Treatment is challenging, given that despite the extensive arsenal, recurrence is common. The present systematic review compares the success rates of therapeutic strategies, aiming to provide a substrate for the right choice in the treatment of the disease in children and adolescents. Methods: Asystematic review was performed based on the guideline for Preferred Report Items for Systematic Reviews and Meta-Analyses (PRISMA), registered on the PROSPERO Platform under identification CRD42021262090. Results: 211 studies were found in thedatabasesconsulted, and 30 met the proposed criteria established in the systematic research. These passed data extraction for qualitative synthesis, demonstrating the effectiveness of non-invasive to invasive methods. Conclusions: Due to good results in disease resolution and reduced adverse effects, the most popular therapy among the authors was Imiquimod. The research called attention to the need for more robust studies and more complex methodologies about the therapeutic approaches of Condyloma Acuminatum in children and adolescents. (AU)

Resumen Objetivo: Describir la experiencia en cirugía electiva de condilomas anales en pacientes mayores de 15 años en un hospital terciario de la Región Metropolitana. Material y Método: Estudio observacional retrospectivo y descriptivo, en el cual se analizan las intervenciones quirúrgicas electivas realizadas entre 2008 a 2021. Resultados: Dentro del período analizado se obtuvo 165 cirugías de condilomas anales, lo que corresponde a 137 pacientes en total. El 85% de los pacientes son de sexo masculino, el 68% de los pacientes son VIH positivo, un 87% de los pacientes MSM (hombres que tienen sexo con otros hombres) son VIH positivo, el 34% de los pacientes tiene antecedente de ETS, el 46% de los pacientes recibió terapia tópica como tratamiento preoperatorio o posoperatorio. Un 25% de los pacientes presenta recidiva en su historia personal, un 21% de los pacientes presenta lesiones anales intraepiteliales de alto grado, un 6% presenta carcinoma escamoso infiltrante. No hubo mortalidad descrita. Discusión: El presente estudio, describe la experiencia en cirugía de condilomas de un hospital terciario de la Región Metropolitana de Chile, cuya población corresponde a un estrato socioeconómico medio y bajo. Se logra describir a la población que es intervenida de condilomas acuminados, además de sus resultados quirúrgicos precoces y a largo plazo. Conclusión: El presente estudio, presenta una población de 137 pacientes operados de condilomas anales, a partir de los hallazgos de la cirugía. Se cumple el objetivo del estudio de caracterizar en un período de 12 años los resultados quirúrgicos de dicha serie, algo no reportado previamente en la literatura chilena.


Objective: To describe the experience in elective surgery for anal condylomas in patients over 15 years of age in a tertiary hospital in the Metropolitan Region. Materials and Method: Retrospective and descriptive observational study about elective surgical interventions performed between 2008 to 2021. Results: Within the analyzed period, 165 anal warts surgeries were obtained, corresponding to 137 patients. 85% of the patients are male, 68% are HIV positive, 87% of the MSM patients are HIV positive, 34% of the patients have a history of STDs, 46% of the patients received topical therapy as preoperative or postoperative treatment. 25% present recurrence in their personal history, 21% present high-grade anal intraepithelial lesions, 6% present infiltrating squamous carcinoma. There was no reported mortality. Discussion: The present study describes the experience in condyloma surgery in a tertiary hospital in the Metropolitan Region of Chile, whose population corresponds to a medium and low socioeconomic stratum. It is possible to describe the population that undergoes surgery for this reason, in addition to its early and long-term surgical results. Conclusion: The present study presents a population of 137 patients operated on for anal condylomas, based on the findings of the surgery. The objective of the study to characterize the surgical results of this series over a 12-year period is fulfilled, something not previously reported in the Chilean literature.

Introdução: O Condiloma uretral é uma infecção sexualmente transmissível comum associada ao vírus do papiloma humano. O acometimento na uretra é raro, principalmente quando trata-se de um paciente HIV positivo com carga viral indetectável. Objetivo: Tem como objetivo discutir sobre o acometimento uretral do papiloma vírus e as possíveis formas de tratamento, assim como revisar a literatura. Relato de caso: O paciente em questão apresentou suspeita de carcinoma de células de transição, sendo encaminhado para tratamento, porém afastado essa suspeita e fechando diagnóstico para lesão condilomatosa uretral, sendo realizado tratamento por ressecção endoscópica das lesões com energia monopolar. Discussão: O acometimento uretral ocorre em até 5% dos casos, e o mesmo apresenta alta probabilidade de malignização. Não existem diretrizes específicas sobre o manejo do condiloma intra-uretral, pois trata-se de uma patologia rara. O objetivo dos tratamentos é remover as verrugas e induzir períodos livre de lesões, sendo que o conservador com crioterapia e o ácido tricloroacético nem sempre são aplicáveis à uretra devido ao difícil acesso. A ressecção endoscópica das lesões tem se mostrado mais eficaz. Conclusão: Urologistas e patologistas devem estar cientes dessa rara possibilidade, a fim de evitar erros de diagnóstico acerca dessa entidade predominantemente benigna. Desta forma apresentamos um caso de ressecção endoscópica de extensa lesão condilomatosa em uretra peniana de uma paciente masculino hemofílico e HIV positivo. [au]


Introduction: Urethral condyloma is a common sexually transmitted infection associated with the human papillomavirus (HPV). Affection in the urethra is rare, especially when dealing with an HIV positive patient with an undetectable viral load. Objective: It aims to discuss the urethral involvement of the papillomavirus and the possible forms of treatment, as well as to review the literature. Case report: The patient in question presented suspicion of transitional cell carcinoma, being referred for treatment, but this suspicion was removed and the diagnosis of urethral condylomatous lesion was closed, with treatment by endoscopic resection of the lesions with monopolar energy. Discussion: Urethral involvement occurs in up to 5% of cases, and it has a high probability of malignancy. There are no specific guidelines on the management of intra-urethral condyloma, as it is a rare condition. The goal of treatments is to remove warts and induce injury-free periods, the conservative with cryotherapy and trichloroacetic acid not always being applicable to the urethra due to difficult access. Endoscopic resection of the lesions has been shown to be more effective. Conclusion: urologists and pathologists should be aware of this rare possibility, in order to avoid diagnostic errors about this predominantly benign entity. Thus, we present a case of endoscopic resection of an extensive condylomatous lesion in the penile urethra of a male patient with hemophilia and HIV positive. [au]

ABSTRACT

Objectives: To compare morphological abnormalities on anal colposcopy against histology to determine anal high-grade squamous intraepithelial lesions (HSILs). Methods: This is a retrospective data assessment of HIV-negative and HIV-positive patients undergoing outpatient follow-up. The sample comprised 54 patients presenting acetowhite lesions on anal colposcopy. Acetowhite lesions were classified according to their morphology into punctation, verrucous, mosaic, ulcerated, or hypervascularized, and biopsies of these specimens were classified as anal HSIL, low-grade squamous intraepithelial lesion (LSIL), or normal. The data were analyzed using SPSS forWindows version 13.0 (SPSS Inc., Chicago, IL, USA). The results were analyzed using the nonparametric Mann-Whitney test, the Fisher exact test and the chi-squared parametric test. A 95% confidence interval (CI) was used and a level of significance <5% was adopted for all statistical tests. Results Fifty-four patients (50 males, 80% HIV+) with biopsied acetowhite lesions were assessed. There were 31 punctation lesions, 1 classified as HSIL (3.2%; 95%CI: 0- 40.0), 17 verrucous lesions, 3 HSIL (17.7%; 95%CI: 0-10.7), and 1 ulcerated, classified as HSIL (100%), and 4 mosaic and 1 atypical vessel lesion, all classified as LSIL. The results showed no association of presence of anal HSIL with positivity for HIV infection or with counts above or below 500/µl in HIV+patients. Statistical analysis was performed using the Mann-Whitney nonparametric test, the Fisher exact test, and the chi-squared parametric test. Conclusion: The comparison of morphological findings on anal colposcopy against histology revealed no morphological pattern suggesting anal HSIL. (AU)

Introducción: Las infecciones de transmisión sexual constituyen un relevante problema de salud a nivel mundial; particularmente afectan la salud sexual de las mujeres ocasionando dolencias y efectos muy negativos, como la infertilidad, los embarazos ectópicos y el cáncer cervicouterino. Objetivo: Describir las características epidemiológicas de las infecciones de transmisión sexual relevantes en mujeres de Santiago de Cuba. Métodos: Se realizó un estudio descriptivo y retrospectivo en la provincia de Santiago de Cuba de la población femenina que presentó infecciones de transmisión sexual durante el período 2007-2017, detallando cada afección según año, municipio, edad, entre otras variables de interés. Asimismo, se analizaron la frecuencia, las tasas de incidencia, las tendencias y las variaciones temporales. Resultados: Las entidades clínicas predominantes fueron el síndrome de dolor pélvico y la secreción vaginal, en tanto, las menos frecuentes resultaron ser el herpes genital y la blenorragia. En el análisis por cada tipo de infección se observó una mayor afectación en féminas de edades jóvenes. Conclusiones: Las infecciones de transmisión sexual constituyen un serio problema de salud en mujeres de la provincia de Santiago de Cuba, que deviene causa importante de embarazos ectópicos y esterilidad. Sin embargo, se debe mejorar la vigilancia epidemiológica de estas enfermedades, pues existe un marcado subregistro de algunas y, por ende, una inadecuada atención, sobre todo en el sexo femenino.


Introduction: Sexually transmitted infections constitute an outstanding health problem worldwide; they particularly affect the sexual health of women causing ailments and very negative effects as infertility, ectopic pregnancies and cervical uterine cancer. Objective: To describe the epidemiologic characteristics of the outstanding sexually transmitted infections in women from Santiago de Cuba. Methods: A descriptive and retrospective study was carried out in the province of Santiago de Cuba in the female population that presented sexually transmitted infections during 2007-2017, with details of each affection according to year, municipality, age, among other variables of interest. Also, frequency, rates of incidence, tendencies and temporary variations were analyzed. Results: The predominant clinical entities were the pelvic pain syndrome and vaginal secretion, as long as, the less frequent were genital herpes and blennorrhagia. In the analysis for each type of infection a higher affectation was observed in young females. Conclusions: Sexually transmitted infections constitute a serious health problem in women from the province of Santiago de Cuba that becomes important cause of ectopic pregnancies and sterility. However, the epidemiologic surveillance of these diseases should be improved, because there is a marked subrecord of some of them and, therefore, an inadequate care, mainly in the female sex.

Resumen INTRODUCCIÓN: El tumor de Buschke-Löwenstein se asocia con el virus del papiloma humano por lo que se considera una infección de transmisión sexual. Se caracteriza por un condiloma gigante, verrugoso, exofítico, con forma de coliflor, de crecimiento lento, pero que durante el embarazo puede crecer rápidamente y formar condilomas solitarios que afectan las estructuras vecinas. CASO CLÍNICO: Paciente de 18 años, primigesta, con 35 semanas de embarazo, sin antecedentes de importancia para el padecimiento actual. Cuatro meses previos inició con verrugas en el perineo, con aumento acelerado de su tamaño en los últimos 30 días, asociado con dolor intenso a la movilización, secreción fétida, eritema e irritación interglútea y formación de un tumor friable con tendencia al sangrado y áreas de necrosis. CONCLUSIONES: El tumor de Buschke-Löwenstein es una alteración poco frecuente causada por el virus del papiloma humano, que puede tener crecimiento acelerado por el estímulo hormonal, propio del estado gestacional. El tratamiento de primera línea es la resección quirúrgica. El seguimiento de las pacientes debe ser estrecho, debido al alto índice de recidiva.


Abstract BACKGROUND: Buschke-Löwenstein tumor is associatted with the human papillomavirus (HPV), which is considered a sexually transmitted infection, characterized by the presence of a giant wart, exophytic condyloma with the shape of a cauliflower, slow growth, but during pregnancy it can grow fast as a solitary condyloma, affecting other structures. This case provides a better understanding of an unusual pathology, which with surgical treatment was obtained aesthetic results and with adequate functionality of external genitalia. CLINICAL CASE 18-year-old female, primiparous with 35 weeks pregnant, with no significant history, 4 previous months begins with the presence of warts in the perineal region with accelerated increase in size in the last 30 days, associated with intense pain on mobilization, fetid discharge, and area oferythema and intergluteal irritation, friable tumor with a tendency to bleed with areas of necrosis. CONCLUSIONS: The Buschke-Löwenstein tumor is a rare pathology caused by HPV, which can present accelerated growth due to the hormonal stimulus of the gestational state, this tumor does not resolve spontaneously, so the surgical approach is considered top of the line. The follow-up of these patients must be close due to the high rate of recurrence.

Summary@#Secondary syphilis is a rare infectious sexually transmitted disease caused by Treponema pallidum in present era. It affects skin as well as other organs of the body. We hereby present a case of an adult male who presented with a one-month history of multiple brownish red maculopapular lesions all over the skin of the body involving the palms, soles, oral cavity and genitalia. His serology was positive for HIV, VDRL and TPHA with a low CD4 count. The patient was treated with three weekly doses of parenteral Benzathine penicillin G, antiretroviral therapy and podophyllin for condyloma acuminata to which he responded well.

INTRODUCCIÓN: Los condilomas son la infección de transmisión sexual más frecuente en el mundo y tiene estrecha relación con la infección por VIH. La topografía y la serología son variables, y algunos presentan mayor riesgo de malignización. OBJETIVO: Describir la prevalencia, características clínicas, topográficas y factores asociados a la presencia de condilomas ano-genitales en pacientes con infección por VIH/SIDA en Guanajuato. MATERIAL Y MÉTODOS: Estudio descriptivo y transversal realizado entre junio y diciembre de 2014. Se incluyeron pacientes adultos con infección por VIH/SIDA, de ambos sexos, del CAPASITS León, México. Se aplicó un cuestionario que incluyó: datos de identificación, y factores de riesgo relacionados a los condilomas ano-genitales, y se les revisó dicha región corporal en busca de condilomas. Se realizó estadística descriptiva de acuerdo al tipo y distribución de las variables y se compararon las medianas de linfocitos T CD4 (LTCD4+) de los pacientes, con y sin condilomas, con la prueba U- Mann-Whitney. RESULTADOS: Se incluyeron 213 pacientes con infección por VIH/SIDA, 181 (85%) hombres. La prevalencia de condilomas ano-genitales fue de 30% (IC95%:23-36%), con mayor frecuencia en la región anal, con 21% de los casos. Encontramos una diferencia significativa entre las medianas en el recuento de linfocitos LTCD4+ en los pacientes, con y sin condilomas, (425 vs 510 LTCD4+/mL, p = 0,034). CONCLUSIONES: Se obtuvo una prevalencia alta de pacientes con condilomas ano-genitales. La presencia de condilomas fue mayor en pacientes con menor recuento de linfocitos LTCD4+. Considerando la elevada prevalencia, se recomienda la aplicación de la vacuna en este grupo de pacientes.


BACKGROUND: Condylomas are the most frequent sexually transmitted infection worldwide, and thus, have a close relation to HIV infection. The site and serology vary, and some pose higher risk of malignancy. AIM: To describe the prevalence, characteristics and associated factors to the presence of anogenital condylomas in patients with HIV/AIDS. METHODS: Descriptive cross-sectional study conducted in the period from June to December 2014. Adult HIV/AIDS positive patients from "CAPASITS" in Leon, Mexico, from both sexes were included. A questionnaire was performed that included: identification data, and risk factors related to anogenital condylomas. Anogenital area was examined for condylomas. Descriptive statistics were carried out according to the type and distribution of the variables, and the CD4 lymphocyte medians of patients with and without condylomas were compared using U- Mann-Whitney test. RESULTS: A total of 213 HIV/AIDS patients were included; 181 (85%) were male. The prevalence of anogenital condylomas was 30% (IC95%: 23-36%), predominating in the anal region, observed in 21% of the cases. A significant difference was found between median CD4 cell count of patients with and without condylomas (425 vs 510 CD4/mL, p= 0.034). CONCLUSION: A high prevalence of patients with anogenital condylomas was obtained. The presence of condylomas was higher in patients with lower CD4 count. Considering the high prevalence, the application of the vaccine is recommended in this patient group.

A infecção genital pelo papilomavírus humano ocorre em 65% das vezes após contato com pele e mucosa contaminadas. A maioria das pessoas elimina o vírus através do sistema imune. Fatores individuais, como tabagismo, idade, multiparidade, uso prolongado de contraceptivos hormonais, comportamento sexual e infecção por outras doenças sexualmente transmissíveis, ocasionam perpetuação viral e lesões genitais, em sua maior parte, assintomáticas. As regiões mais afetadas são anogenitais. O tratamento deve ser individualizado. O tratamento agressivo pode ocasionar distúrbios cicatriciais. Relatamos um caso de múltiplos tratamentos para retirada de condiloma genital com consequente formação de extenso queloide vulvar


Genital infection by Human Papillomavirus occurs 65% of the time after contact with contaminated skin and mucosa. Most people eliminate the virus through the immune system. Individual factors such as smoking, age, multiparity, prolonged use of hormonal contraceptives, sexual behavior, and infection by other Sexually Transmitted Diseases cause viral perpetuation and genital lesions, most of them asymptomatic. The most affected regions are anogenital. Treatment should be individualized. Excessive treatment can lead to scarring. Objective@#To investigate the distribution of the gene subtypes of human papillomavirus (HPV) in male patients with condyloma acuminatum (CA) and analyze the characteristics of the gene subtypes.@*METHODS@#We extracted genomic DNA of the HPV virus from the genital tissue of 70 male CA patients, detected the DNA subtypes of HPV using the PCR-reverse dot hybridization technique, and analyzed the rates of different subtypes identified and their characteristics of distribution in different age groups.@*RESULTS@#The male HPV-positive patients were mainly infected at the age of 20-39 years, primarily with high- and low-risk mixed infection of various subtypes, which accounted for 61.54% in the 20- to 29-year-olds and 42.86% in the 30- to 39-year-olds. Among the 70 CA patients, 22 HPV subtypes were identified, the top five subtypes including HPV 11 (21.08%), HPV 6 (19.46%), HPV 42 (6.49%), HPV 59 (6.49%) and HPV 53 (5.95%); 20 infected with a single subtype (28.57%), 19 with two subtypes (27.14%) and 31 with three or more (44.29%); and 30 infected with a low-risk single subtype (42.86%) and 40 with both high- and low-risk multiple subtypes (57.14%).@*CONCLUSIONS@#Male patients with CA are mainly infected with HPV 11 and HPV 6, with a significantly higher rate of multi-subtype than single-subtype infection, and the multi-subtype patients chiefly with high- and low-risk mixed infection. Men aged 20-39 years old are most commonly affected by CA.

ABSTRACT Host immunogenetic setting is involved in the regulation of human papillomavirus (HPV) infection and development of condyloma acuminatum (CA). We investigated the correlation of two common single nucleotide polymorphisms (SNPs) (−607C/A and −137G/C) of IL-18 with the susceptibility of CA in a large Chinese cohort. Out of 408 CA patients analyzed, 300 had HPV infection transmitted through sexual contact (SC) and 108 through non-sexual contact (NSC). In addition, 360 healthy volunteers were enrolled as controls. SNPs at positions −607C/A and −137G/C in IL-18 promoter were analyzed. Comparing CA patients to healthy controls, no dominant relevance was found between the IL-18 promoter −607 C/A or −137G/C polymorphisms and the CA disease either identified genotypically (p > 0.05) or by allelically (p > 0.05). However, the IL-18 promoter −137G/C polymorphism genotype and allele frequencies in the NSC CA group, but not between in the SC group, were significantly higher than in the controls. There was no dominant relevance between IL-18-607C/A polymorphism genotype and allele frequencies among SC, NSC CA patients, and controls. Our study demonstrates that polymorphism −137G/C in IL-18 promoter is significantly correlated with risk of CA in NSC patients.

Introduction: Some viral warts are refractory to treatment, some others tend to recur. Oral isotretinoin is useful against warts to varying degrees. Objective: To determine the efficacy of oral isotretinoin for treating mucocutaneous human papillomavirus infections. Methods: A systematic review and meta-analysis of studies published from the date of inception of the databases to December 30, 2017 were conducted. Randomized controlled trials or case series with ≥10 patients with mucocutaneous human papillomavirus infection who had received oral isotretinoin treatment were analyzed. The meta-analysis estimated the pooled odds ratio and pooled response rate. Results: The review included eight studies. Trials of oral isotretinoin versus placebo treatment revealed that isotretinoin effectively treated mucocutaneous human papillomavirus infections (odds ratio: 43.8, 95% confidence interval: 9.7–198.8). The pooled estimate of the complete response rate of oral isotretinoin to mucocutaneous human papillomavirus was 67.7% (95% confidence interval: 49.5–81.7%). Another pooled estimation revealed that 83.9% (95% confidence interval: 59.7–94.9%) of patients exhibited at least 50% lesion clearance, whereas 12.3% with complete response experienced recurrence. Limitations: This meta-analysis had a small sample size and high inter-study heterogeneity. Conclusion: Oral isotretinoin is superior to placebo for treating mucocutaneous human papillomavirus infections, particularly plane warts. The recurrence rate and risk of severe side effects are low.

Condyloma acuminata, the most common sexually transmitted viral disease of genitalia is caused by Human Papilloma Virus (HPV). Methods: The present work was designed as a randomized interventional study during the period from November 2016 to October 2017. Results: A total of 80 non pregnant sexually active female patients were included in the study. In this study the mean age of the patients was 23 years. Out of total 80 patients, 67 had affected partners. The mean duration of complete clearance was 3.6 weeks for podophyllin, 4.8 weeks for cryotherapy, 2 weeks for C02 laser and 1.8 weeks for CO2 laser + cryotherapy. In CO2 + cryo group, complete clearance was seen in all 20 lesions, in CO2 alone group 17 lesions, cryo alone showed complete clearance in 13 patients and in podophyllin group the number was 15. Conclusion: This study shows that CO2 laser + cryotherapy is most effective treatment followed by CO2 laser alone and podophyllin.

Condyloma acuminatum (CA) is a benign tumor primarily caused by infection with human papillomavirus (HPV) type-6 or type-11, lesions of which are most frequently found on the genital, perianal squamous mucosa, and skin. CA outside the anogenital region is not common. Here, we report a case showing simultaneous presence of CA on the vaginal orifice, areola, groin, and umbilicus in a 32-year-old pregnant woman. Histopathological examination and HPV detection are essential in making a definitive diagnosis of CA on multiple sites. The patient was treated with microwave and liquid nitrogen cryotherapy, but repeated relapse of the lesions were observed. Patients with CA during pregnancy represent a special risk group. These cases are usually characterized by fast-growing warts, multi-site or special site warts, a reduced tolerance, and poor response to treatment. Hence, we should pay more attention to CA in pregnant women in the process of diagnosis and treatment, especially cases outside the anogenital region.

El condiloma acuminado es una enfermedad de transmisión sexual causada por el virus del papiloma (VPH). Se encuentra con frecuencia en la región ano-genital y es poco frecuente en la cavidad bucal. Sin embargo, el aumento de la actividad sexual oral ha favorecido aún más la aparición de PVH en la mucosa oral. Este informe describe un condiloma en la cavidad oral (lengua) de un hombre de 43 años con antecedentes de práctica sexual oral, cuyo diagnóstico se confirmó mediante una biopsia.


The acuminated condyloma is a sexually transmissible disease caused by the Papilomavirus (HPV). It is frequently found in the anus-genital region and rare in the mouth cavity. However, the increase of the oral-sexual activity has even more favored the occurrence of the PVH in the oral mucous. This report describes a condyloma I the oral cavity (tongue) of a 43 year old man with a history of oral sex practice, which diagnostic was confirmed by a biopsy.

Objective@#To analyze the clinical features of perianal condyloma acuminatum in HIV infected men who have sex with men (MSM) and assess the effects of combined treatment of microwave and photodynamic therapy.@*Methods@#Clinical and laboratory data of 260 male patients with perianal condyloma acuminatum were collected and microwave combined with photodynamic therapy was applied on them.@*Results@#The majority of the patients with perianal condyloma acuminatum were young people (243/260). Warts were mainly located in the anus or from the outside to inside of the anus (245/260). Among them, the proportion of warts position of extraanal in HIV-infected group (8/220) was lower than that of non-HIV-infected group (7/40). HPV types 11 (70/118), 6 (44/118) and 16 (40/118) were the most common types of HPV infection. People with HIV infection were more likely to develop high-risk, mixed types HPV infection and multiple HPV infection than those without HIV infection. More than half of the patients (146/260) needed only 1-3 sessions of photodynamic treatments. There was no significant difference in the times of photodynamic treatments and the numbers of photosensitizers required between the HIV-infected and non-HIV-infected groups. The recurrence rate of HIV-infected group (13.6%, 30/220) was also similar to that of non-HIV-infected group (15.0%, 6/40), but far lower than those who underwent microwave therapy alone (45%, 18/40).@*Conclusions@#The combination therapy significantly reduced the recurrence rate of perianal condyloma acuminatum in HIV infected MSM.

ABSTRACT

OBJECTIVE@#To test the acute and chronic toxicity of topical application of 0.5% podophyllotoxin-loaded nanostructured lipid carriers (POD-NLC) to the vaginal mucosa.@*METHODS@#Twelve New Zealand rabbits were randomized into 3 groups and subjected to daily topical applications of normal saline (control group), 0.5% podophyllotoxin tincture (POD-T) or 0.5% POD-NLC on the vaginal mucosa for 10 consecutive days, and the pathological changes in the mucosa were graded using the Eckstein scoring system.The acute toxicity of POD-NLC was tested in 20 SD female rats, which received intravaginal administration of POD-NLC or vehicle for 3 times within 24 h; After 14 days of continuous observation, the rats were dissected for calculating the viscera coefficient.For testing the chronic toxicity of POD-NLC, 80 SD female rats were randomized into 4 groups and subjected to daily intravaginal administration of the vehicle or POD-NLC at low, moderate or high doses for 13 consecutive weeks.The rats were weighed once a week and at the end of the experiment, 2/3 of the rats from each group were sacrificed to collect blood samples, calculate the viscera coefficient, and examine the pathological changes in the liver.The remaining 1/3 rats were observed for another 2 weeks without further drug treatment and the same examinations were performed.@*RESULTS@#In the rabbits, 0.5% POD-NLC elicited only mild irritation while POD-T caused moderate irritation of the vaginal mucosa.In the acute toxicity test, the organ coefficients were comparable between the rats treated with the vehicle and POD-NLC (>0.05).Long-term intravaginal administration of POD-NLC did not produce significant changes in the behavior, activity, body weight, blood biochemical profiles or organ coefficient as compared with the vehicle control group (>0.05).@*CONCLUSIONS@#Intravaginal administration of 0.5% POD-NLC causes very mild irritation without obvious acute or chronic toxicity to the vaginal mucosa in rabbits and rats.
